Sports Physical Clinic at Illini West
SPORTS PHYSICAL WORKSHOPS SET FOR HANCOCK COUNTY STUDENT ATHLETES PARTICIPATING IN THE 2012-2013 SCHOOL YEAR.
Carthage, IL –As this school year is coming to an end, we will soon need to prepare the parents’ to-do lists before the next. Memorial Medical Clinic and Advance Physical Therapy team up to offer two sports physical workshops for Hancock County student athletes. Any Jr. & Sr. High student participating in school sponsored athletic competitions for the upcoming 2012-2013 school year should attend this workshop. This includes football, volleyball, basketball, track, baseball, softball or any other sports that require a sports physical.
“Our number one goal for our patients is safety,” Stephanie Meyers, Memorial Medical Clinic Manager “it is only natural for us to provide this workshop to ensure the safety of our student athletes as well.”
One workshop will be held at Illini West High School on Saturday May 12, 2012, from 8am to 11:30am and one will be held at Hamilton High School on Saturday, June 2, 2012, from 8am to 11:30am. An EKG will be included in the sports physical exam. The cost of the physical exam is $15.00, to be paid in cash or check at the time of the workshop. The $15.00 fee will be donated back to your school’s athletic department.
A parent will need to be present or will need to sign and complete the parent portion of the physical form prior to arriving at the workshop. Forms are available from the athletic office of the child’s school. These sports physical workshops are NOT for regular school physicals that require immunizations. If any immunizations are needed, they will be scheduled during normal office hours and there will be an additional and billable expense to the patient and/or patient’s insurance company. Please bring cash or check with you, debit or credit cards and insurance claims will not be able to be processed. We will not perform an exam if these two items are not complete.
“This gives the student athletes a convenient opportunity to undergo an essential and often procrastinated element for the upcoming school year before the hustle and bustle of summer and back-to-school activities surmount,” says Dr. Kurt Davis. “Please give this workshop thoughtful consideration as a way to give back to your school district and prepare for the next year’s athletics,” adds Davis.
