Swing Bed
Skilled nursing services as well as rehabilitative services are available through our Swing Bed Program located within the Medical/Surgical care. Our Case manager and our Discharge Planner would be happy to discuss these services as appropriate.

What is Swing Bed?
The Swing Bed program is really just a name for the provision of skilled services.  The skilled level of care services are provided after patients have had an acute care stay.  Some places called a skilled unit “transitional care” because it is often a place to transition from the hospital to home.  At Memorial Hospital, skilled services are offered in “swing beds”.  Care is provided in the Medical-Surgical unit and the bed “swings” to a different level of care.

Services typically offered to patients at the skilled level are therapies designed to rehabilitate or re-condition.  Often in the elderly, an illness will leave the patient weakened.  By offering therapy with the goal of returning the patient to their previous level of functioning, we best serve our customers.

Other services provided in the Swing Bed setting include medication administered intravenously (IV) or nutrition provided by tube feeding or IV.  Complex wound care or pain management with IV medications may also require the skilled setting.

The Swing Bed team works cooperatively to plan the care and assist with education and discharge planning.  The team discusses services necessary for post-discharge.  Care is coordinated with the patient, the family and physician to best meet the discharge needs.

The Swing Bed program at Memorial Hospital is an important service that allows patients to recover more fully before discharge.

The physician's (or radiology or pathology, etc.) services at Memorial Hospital are provided by independent practitioners. Unless specifically indicated otherwise, these practitioners are not employees or agents of Memorial Hospital.
